How to begin Counseling:
​Before we can set you or a family member up with one of our counselors, there are a few things that we need, and which forms are required depend on who is seeking services.

Please contact us at 806-342-2500 to schedule an intake appointment.

For your intake assessment, we will need:
•Basic demographic information
•Your Insurance card, Medicaid or CHIP card (if applicable)
•Referral from community agency or healthcare provider (if applicable)
To qualify for our sliding fee scale, please bring verification of income for everyone living in the household, consisting of:
3 paycheck stubs or a paycheck stub with year to date (for those who are employed)
OR SSI letter (for those receiving SSI)
OR Any other documentation that verifies your income (ex: most recent income tax return).
Upon receipt of required documentation or verification, you will have an appointment scheduledwith one of our highly-trained counselors.

Your first counseling appointment will be set up for the soonest available opening. Please arrive 15 minutes early to your initial appointment to allow time to complete necessary paperwork.

Payment Policy:
Clients are asked to pay for their counseling appointments prior to the session beginning. The cost of counseling is determined on a sliding fee scale based on income. Self-pay, insurance, Medicare and Medicaid are also accepted. All appointments are subject to a 24-hour cancellation policy; cancellation with less than a 24-hour notice will result in a charge of your session fee.
